What does My Care Checklist include?
My Care Checklist includes exams, tests and screenings recommended just for you. You can check off visits once you’ve completed them. You’ll be reminded about what’s coming up so you’re prepared for next steps. Your personalized My Care Checklist includes resources about health topics to keep you informed, making it easy to stay on top of preventive care.
Make preventive care a must-do on your checklist
With a UnitedHealthcare Group Medicare Advantage plan, your preventive care member benefit includes an annual physical and wellness visit. This yearly visit is covered at no additional cost as part of your health plan.*
Whether you have specific questions or are in good health, think of this as your chance to check in with your primary care provider (PCP) and talk about any changes or concerns. Your PCP can help detect problems as early as possible when treatment may be more effective.
Your annual physical and wellness visit will be automatically checked off your My Care Checklist after completion. When you log in to your member account, you’ll also see results of any tests and screenings and next steps.

How to schedule your annual physical and wellness exam
Contact your PCP to schedule your annual physical and wellness visit. For help finding a provider, sign in to your member site and choose Find Care. You can also call Customer Service at the number on your member ID card.
